Abstract

Objective: to characterize the scientific productions on patient safety in Primary Health Care (PHC). Methodology: Scope review that identified studies in PubMed, Scopus and BVS, having as indexed descriptors: "primary health care" and "patient safety", associated through the Boolean operator AND. Seventy-two original articles, published in full text, in the last 5 years, in English, Spanish and Portuguese were selected. Results: articles published between 2014 and 2019, mostly quantitative, whose research subjects were health professionals. Three themes were identified: measures implementations to improve the patient safety culture, describing team care, workflow design, adequate training and remuneration of the professionals, workshops implementation, clear and adequate instructions in medical prescriptions and a plan for crisis; threats to the effectuation of patient safety, highlighting inappropriate use of medications, errors in clinical decision-making and poor communication; and perception of the patient safety culture, bringing the professionals' experience as a defining characteristic. Conclusion: for greater effectiveness, the implementation and strengthening of the patient safety culture in primary care should focus on the professionals' practices and the transformation of the organizational conditions that determine them.
