BENEFITS OF THE ZERO BALANCE ULTRAFILTRATION TECHNIQUE (Z-BUF) DURING CARDIOPULMONARY BYPASS IN PATIENTS SUBMITTED TO HEART SURGERY

Abstract

Cardiopulmonary bypass (CPB) is the temporary replacement of the heart and lungs by a machine during some surgical procedures. The use of crystalloid solutions to fill and remove air from the CPB circuit leads to hemodilution of the patient, generating complications such as reduced hemoglobin levels and reduced oxygen transport and supply, requiring the use of ultrafiltration techniques. Ultrafiltration reduces edema formation and the need for blood transfusions, in addition to controlling the systemic inflammatory reaction by removing pro-inflammatory agents. In zero balance ultrafiltration (Z-BUF), a solution is infused into the CEC circuit and an equal volume of fluid is removed via the ultrafiltration column. This process allows small molecules to be removed from the blood, while cells and large proteins remain in the circulation. Z-BUF was developed to reduce inflammatory mediators associated with CPB, although it currently helps correct electrolytic abnormalities that occur during CPB surgery. The study elucidates the benefits of Z-BUF used in CPB in patients undergoing cardiac surgery. It is observed that Z-BUF brings numerous benefits to the patient undergoing CPB during cardiac surgery, including reduction of inflammatory factors, postoperative blood loss, intubation time and ICU stay, in addition to maintaining a most adequate and physiological electrolytic balance for the body. Therefore, it is concluded that Z-BUF is able to provide benefits to the patient, in order to mitigate the damage caused by the use of CPB during cardiac surgery.
